A flash based retail consumer survey created by me for the marketing team at CACI.
The survey was built in flash, with the brief for all 5 questions to be on the same page, making it appear quick and easy to use.
I used some easing functions and actionscript for scroll bars / logic etc, and asp.net to pass the details from the survey onwards.
THe content is dynamic to allow the survey to be repopulated, and re-used with the minimum of re-work.
